Qualification applicationLast modified: 10-06-2002
(Information for people who have completed their 1st, 2nd or 3rd cycle studies)
Procedure for applying for the qualification (at the School's central office (1st and 2nd cycle), or the central office of the department/unit where you have carried out the studies or where they are managed (3rd cycle)) 1. You will have to fill in the Qualification Application Form that your central office will give you once your personal transcript is reviewed. Personal details Please put your name and surnames as they appear on you current national identity document (DNI) or residency permit (for foreigners) without abbreviations. Include accents and "i" or "y" between surnames where applicable. Any exceptions will have to be accredited with an official Registry Office document (birth certificate or foreign equivalent). People born abroad will have to state the appropriate federal state, canton, department or region in the "department (foreigners)" box, if applicable. Qualification Please state the complete name of the qualification you are applying for and, if applicable, the major. 2. You must submit the following documentation at the central office: - A photocopy of your current DNI, passport or residency permit (foreigners), if it is different from the last copy you submitted. If the document is out of date you will have to bring it, along with proof of having asked for a new one. The sooner you provide us with a photocopy of the new document the better, since it is necessary for you to obtain your qualification. 3. You must pay the fees for the issuing of the qualification as instructed by the central office. The receipt for this payment will have the same official validity as the qualification itself until it is issued. You can ask for a substitute document certificate (proof that the qualification is being processed) if the receipt is insufficient for your application or if you have to present it to, for example, foreign government bodies.
Procedure for collecting the qualification You must collect the qualification personally at the central office, within five years of receiving notification that the qualification is ready. You must bring your DNI, passport or residency permit (foreigners), and the receipt for the qualification issuing fees. Please bring a photocopy of this document if it is different from the last copy you submitted. If you cannot collect it in person, you can authorise someone else to do so, by written permission. If you live in an area where the University has no offices, you can give written permission for your qualification to be sent to an official office near you, such as the provincial education and culture office, that of your autonomous community or the closest consulate to where you live. Copies For copies due to loss, deterioration, updating, change of name/surname, change of nationality, etc., please contact the central office. Any other issues regarding the qualification For any other issues regarding the qualification, please contact the central office. Important: Qualifications are issued in a bilingual format: both Spanish and Catalan on the same document. (You must always bring the original documents for the photocopies to be validated officially) UPC - February 1999 | ||
